Home visits may include

- Selected sick visits
- Medication review
- Chronic condition follow-up
- Post-treatment follow-up
- Selected preventive services
- Coordination of testing and additional care when needed

- Patient requests a home visit.
- FirstWay Health confirms location and medical appropriateness.
- An available time is scheduled.
- The provider performs the visit and develops the care plan.
